PREFERRED PROVIDER ORGANIZATION (PPO)Both the Co-pay and deductible plans provide comprehensive major medical benefits for inpatient and outpatient physician services. The only difference between the
two plans is that with the traditional deductible plan, you must pay your
deductible up to whatever limit you choose before medical coverage is provided.
While with the Co-pay plan, you have a set amount you pay per office visit
